The first film shows an opacity that is either a pneumonia, old scarring or some other phenomenon, and the second film does not show that. The difference in films, in terms of color, has more to do with exposure-that is, the second film is much more exposed than the first, making it much darker. This may give the false impression that the lungs are much clearer. The only difference between the first and second film really is the absence of the right upper lobe density (which is not tar buildup).
