Gastroesophageal Reflux disease is mainly muscular and yes light bleeding can occur in acute cases but in his words there was blood all over the bowl, I read this as a substantial amount which leads me to a vascular issue or serious ulceration, torn/ruptured esophagus (common in those who vomit often).
